How does Medical Team Training affect operating room performance. What are the key outcomes of implementing MTT in surgical settings. Can MTT reduce delays and improve team communication in the OR. How does MTT impact patient safety and surgical outcomes.
Understanding Medical Team Training (MTT) in the Operating Room
Medical Team Training (MTT) has emerged as a crucial strategy for enhancing teamwork and patient safety in operating rooms (ORs). This comprehensive approach aims to improve communication, coordination, and overall performance among surgical teams. But what exactly does MTT entail, and how effective is it in practice?
MTT typically involves a structured program that focuses on developing teamwork skills, improving communication, and fostering a culture of safety among OR personnel. The training often includes simulations, debriefings, and the implementation of standardized protocols to ensure consistency in team performance.
Key Components of Medical Team Training
- Intensive one-day training sessions
- Development of standardized briefing and debriefing protocols
- Implementation of perioperative routines
- Documentation of OR miscues and delays
- Case scoring system for team performance evaluation
Measuring the Impact: A Comprehensive Analysis of 4,863 Cases
To assess the effectiveness of MTT, a large-scale study was conducted, analyzing 4,863 MTT debriefings. This extensive dataset provided valuable insights into the tangible benefits of implementing MTT in surgical settings. The study utilized a multifaceted approach to evaluate the impact of training on various aspects of OR performance.
Study Methodology
- Baseline data collection prior to MTT implementation
- Follow-up assessments at 12 and 24 months post-training
- Analysis of case delays, scores, and specific perioperative issues
- Administration of safety attitudes questionnaires
By comparing pre- and post-training data, researchers were able to quantify the improvements resulting from MTT implementation. This comprehensive approach allowed for a nuanced understanding of how MTT affects various aspects of OR performance over time.
Significant Reductions in Operating Room Delays
One of the most striking outcomes of the MTT implementation was the substantial reduction in case delays. Prior to training, 23% of cases experienced delays. However, just one year after MTT implementation, this figure dropped dramatically to 10%. Even more impressive, this improvement was sustained at the 24-month mark, demonstrating the long-term efficacy of the training program.
How did MTT contribute to such a significant reduction in delays? The training program likely improved team coordination and communication, allowing for more efficient pre-operative preparations and smoother transitions between cases. Additionally, the standardized protocols implemented as part of MTT may have helped identify and address common sources of delays more effectively.
Breakdown of Delay Reductions
- Preoperative delays decreased from 16% to 7%
- Equipment-related issues and delays dropped from 24% to 7%
- Hand-off issues were reduced from 5.4% to 0.3%
Improved Case Scores and Team Performance
In addition to reducing delays, MTT had a positive impact on overall case scores, which serve as a measure of team performance. The mean case score increased from 4.07 to 4.87 on a 5-point scale one year after training, with this improvement maintained at the 24-month follow-up.
What factors contributed to the increase in case scores? The improvement likely stems from enhanced team communication, better coordination of tasks, and a more cohesive approach to patient care. The standardized briefing and debriefing processes implemented as part of MTT may have played a crucial role in aligning team members’ expectations and goals for each procedure.
Notable Improvements in Case Scores
- Cases with low scores (below 3) decreased from 23% to 3%
- Overall mean case score increased by 0.8 points
- Sustained improvement observed at both 12 and 24 months post-training
Enhanced Adherence to Clinical Guidelines
The study also revealed that MTT had a positive impact on adherence to important clinical guidelines. Specifically, there was a marked improvement in the timing of prophylactic antibiotic administration, a critical factor in preventing surgical site infections.
Prior to MTT implementation, adherence to antibiotic timing guidelines stood at 85%. Following the training, this figure increased to 97%, representing a significant enhancement in patient safety protocols. This improvement underscores the potential of MTT to not only enhance team performance but also directly impact patient outcomes.
Factors Contributing to Improved Guideline Adherence
- Increased awareness of the importance of timely antibiotic administration
- Better communication between anesthesia providers and surgical team members
- Standardized protocols for preoperative patient preparation
- Regular review and reinforcement of best practices during team debriefings
Identification and Resolution of Systemic Issues
One of the unexpected benefits of implementing MTT was the identification of a major systems issue related to perioperative medication orders. Through the debriefing process and the multidisciplinary MTT committee reviews, this problem was not only identified but also successfully corrected.
How did MTT facilitate the discovery and resolution of this systemic issue? The structured debriefing process likely created a safe space for team members to voice concerns and report recurring problems. Additionally, the multidisciplinary nature of the MTT committee allowed for a comprehensive approach to problem-solving, bringing together expertise from various aspects of perioperative care.
Steps in Addressing Systemic Issues
- Identification of the problem through debriefing sessions
- Review by the multidisciplinary MTT committee
- Development of a corrective action plan
- Implementation of changes to perioperative medication order processes
- Monitoring and evaluation of the effectiveness of the solution
Improved Perceptions of Teamwork and Patient Safety
Beyond the quantitative improvements in performance metrics, the study also revealed positive changes in team members’ perceptions of teamwork and patient safety. Safety attitudes questionnaires administered at baseline and one year after MTT implementation showed significant improvements in these areas.
What factors contributed to these improved perceptions? The structured nature of MTT likely fostered a sense of shared responsibility and common purpose among team members. Regular debriefings and open communication channels may have helped build trust and mutual respect within the team. Additionally, the visible improvements in performance metrics could have reinforced team members’ confidence in their collective abilities.
Key Improvements in Team Perceptions
- Enhanced sense of teamwork among OR personnel
- Increased confidence in the team’s ability to handle complex situations
- Greater awareness of patient safety considerations
- Improved communication across different disciplines within the OR
Long-Term Sustainability of MTT Benefits
One of the most encouraging aspects of this study is the demonstration of sustained improvements at the 24-month follow-up. This longevity suggests that MTT can create lasting changes in OR team dynamics and performance, rather than just short-term gains.
How can healthcare organizations ensure the long-term sustainability of MTT benefits? Regular reinforcement of training principles, ongoing debriefing sessions, and continuous monitoring of performance metrics are likely key factors. Additionally, integrating MTT principles into organizational culture and onboarding processes for new team members can help maintain improvements over time.
Strategies for Sustaining MTT Benefits
- Regular refresher training sessions
- Incorporation of MTT principles into daily OR routines
- Continuous performance monitoring and feedback
- Recognition and reward systems for teams demonstrating excellent teamwork
- Integration of MTT concepts into staff development and leadership programs
In conclusion, this comprehensive analysis of 4,863 cases provides compelling evidence for the efficacy of Medical Team Training in improving OR performance. The sustained reductions in delays, improvements in case scores, enhanced guideline adherence, and positive changes in team perceptions all point to the significant impact MTT can have on surgical outcomes and patient safety. As healthcare organizations continue to seek ways to optimize OR efficiency and effectiveness, MTT stands out as a powerful tool for achieving these goals.

The efficacy of medical team training: improved team performance and decreased operating room delays: a detailed analysis of 4863 cases
Objectives:
Medical team training (MTT) has been touted as a way to improve teamwork and patient safety in the operating room (OR).
Methods:
OR personal completed a 1-day intensive MTT training. A standardized briefing/debriefing/perioperative routine was developed, including documentation of OR miscues, delays, and a case score (1-5) assigned by the OR team. A multidisciplinary MTT committee reviewed and rectified any systems problems identified. Debriefing items were analyzed comparing baseline data with 12 and 24-month follow-up. A safety attitudes questionnaire was administered at baseline and 1 year.
Results:
A total of 4863 MTT debriefings were analyzed. One year following MTT, case delays decreased (23% to 10%, P < 0.0001), mean case score increased (4.07-4.87, P < 0.0005), and both changes were sustained at 24 months. One-year and 24-month follow-up data demonstrated decreased frequency of preoperative delays (16%-7%, P = 0.004), hand-off issues (5.4%-0.3%, P < 0.0001), equipment issues/delays (24%-7%, P < 0.0001), cases with low (<3) case scores (23%-3%, P < 0.0005), and adherence to timing guidelines for prophylactic antibiotic administration improved (85%-97%, P < 0.0001). Surveys documented perception of improved teamwork and patient safety. A major systems issue regarding perioperative medication orders was identified and corrected.
Conclusions:
MTT produced sustained improvement in OR team function, including decreased delays and improved case scores. When combined with a high-level debriefing/problem-solving process, MTT can be a foundation for improving OR performance. This is the largest case analysis of MTT and one of the few to document an impact of MTT on objective measures of operating room function and patient safety.

Korea short track champion Shim barred from training with team mates

SEOUL, Oct 12 (Reuters) – South Korea’s double Olympic short track gold medallist Shim Suk-hee has been excluded from training with the national team amid a probe into text messages she sent during the 2018 Games that suggested she may have intended to take down a team mate.
An online newspaper last week revealed messages exchanged between Shim and a coach during the Games in which she ridiculed her team mate Choi Min-jeong and said she might try to create a “female Steven Bradbury” if things were not going her way.
Australian Bradbury won gold in the men’s 1,000m at the 2002 Winter Olympics after all the other competitors crashed at the last turn.
In the 2018 women’s 1,000m final in Pyeongchang, Shim and Choi became tangled up and fell over at the final corner, with Shim being disqualified and Choi finishing fourth.
Yonhap news agency reported on Monday that the Korea Skating Union (KSU) planned to investigate whether there had been any intent on Shim’s part.
The report said the KSU had “separated” her from the national team at a training centre in the central city of Jincheon where they are preparing for qualifiers for the 2022 Beijing Winter Games.
Shim will not be able to compete at the International Skating Union (ISU) Short Track Speed Skating World Cup, which is scheduled to begin in Beijing on Oct. 21 and determines quotas for the upcoming Olympics, the report said.
“We’ve concluded after consulting the athletes, including Shim herself, and coaches that it would be difficult to train together in this situation,” a KSU official was quoted as saying.
The KSU was immediately available for comment.
Shim apologised on Monday while Choi’s management agency has called for an investigation.
“I’d like to sincerely apologise for disappointing and hurting many people with my immature actions and words,” Shim said.
However, she denied taking down Choi on purpose, saying it was “just unfortunate we crashed into each other”.
Choi’s agency issued a statement urging the KSU to provide protection for her and to conduct an investigation into the crash at the Games.

Eagles training camp 2021 observations, Day 12: 1st team OL back together
The Eagles were back at practice on Saturday morning as they prepare to welcome in the Patriots for a couple days of joint practices to start the next work week.
No pads for the Birds on Saturday and it was one of the shortest practices of the summer.
There were still plenty of things to observe:
1. Housekeeping notes:
• The Eagles made some roster moves this morning, activating Le’Raven Clark off the PUP list and waived WR Adrian Killins and TE Caleb Wilson. Clark (Achilles) was at practice Saturday but is in a ramping up period.
• DeVonta Smith (knee) has been upgraded from week-to-week to day-to-day. He was at practice with a helmet on Saturday but did not participate. During individual drills, Smith was on a side field with a trainer and was running sprints.
• The other day-to-day guys: Jason Huntley (ribs), Craig James (foot), Ryan Kerrigan (thumb), K’Von Wallace. Huntley is a new addition to the injury report, while Wallace was upgraded from week-to-week.
• The week-to-week players: Andre Dillard (knee), John Hightower (groin), JaCoby Stevens (hamstring), Davion Taylor (calf), Casey Tucker (biceps).
• Limited today: Grayland Arnold (hamstring), Genard Avery (groin), Jason Croom (knee), Isaac Seumalo (hamstring). Arnold, Avery and Croom all returned to practice on Saturday.
2. Because Seumalo was back in team drills some on Saturday, the Eagles finally had their first-team offensive line for the first time all summer. While Nick Sirianni hasn’t named Jordan Mailata the Eagles’ starting left tackle, it’s pretty obvious he will be. Seumalo and Brandon Brooks didn’t take every rep but they were out there together.
Here was the full starting line:
Left tackle: Jordan Mailata
Left guard: Isaac Seumalo
Center: Jason Kelce
Right guard: Brandon Brooks
Right tackle: Lane Johnson
3. Zach Ertz had a fantastic touchdown catch in the end zone on a perfect pass from Jalen Hurts during 7-on-7 drills. Hurts dropped in the pass over the outstretched arms of Anthony Harris and Ertz made a leaping grab in front of Darius Slay.
Soccer star Julie Ertz (and Zach’s wife) was a spectator at Saturday’s practice.
Other 7-on-7 notes:
During this 7-on-7 red zone period, Hurts was 4/5 with one touchdown. His other completions didn’t result in touchdowns.
Meanwhile, Joe Flacco was 1/4 with a touchdown going to Andre Patton with Michael Jacquet in coverage.
Nick Mullens was 2/3 in the red zone with a touchdown to Marken Michel, who was just brought back the other day.
4. The Eagles defensive line had three sacks in team drills on Saturday:
• Josh Sweat bent around Mailata and picked up a sack. But I liked even more that Sweat came back on the next rep with a stab. Sweat has a great blend of power and quickness.
• The next sack came from Javon Hargrave, who has been having a great summer. He overpowered Seumalo to collapse the pocket quickly.
• The third sack came from Brandon Graham against Lane Johnson. Give an assist to the coverage forcing Hurts to hold the ball, but Graham got the best of Johnson on this rep. Because the Eagles have had their first teams face off so much this summer, Graham vs. Johnson has been an everyday occurrence and it’s fun to watch.
5. After a big touchdown grab in Thursday’s game, Quez Watkins finally got some extended reps with the first-team offense. He was out there for the first play of team drills and caught a pass from Hurts. We saw some work with Watkins in the slot, which is where his touchdown came from on Thursday.
6. Rookie Zech McPhearson made a great leaping pass breakup in team drills on Saturday. McPhearson’s 40 1/2-inch vertical put him in the 92nd percentile among corners in the draft. He showed it off on this play.
7. We saw Hurts miss Watkins with a deep ball in Thursday’s game and on Saturday he missed a deep ball to Jalen Reagor who got a step on Darius Slay. Hurts overthrew Reagor, who didn’t look super clean in the route.
8. On the second rep of team drills, Dallas Goedert settled into a zone and would have had an easy catch on a perfectly thrown ball from Hurts had Slay not timed it perfectly for another PBU.
9. Travis Fulgham had a nice catch leaping backward in the first team period of practice. He hasn’t had a great training camp but he still has the ability to bring down some tough passes. He just needs to do it more consistently.
10. We’ve watched Doug Pederson making the rounds at training camp practices in Chicago and Indianapolis and the Eagles had a guest on Saturday. Former Chargers head coach Mike McCoy made an appearance.
McCoy was the Chargers head coach from 2013-16. During that time, Sirianni was a quality control coach and also coached quarterbacks and receivers.
“Yeah. It’s great to have him here,” Sirianni said. “It’s really great to have him here and see him practice. I always respected how much Mike paid attention to situations. He just — there’s so many situations that pop up in a game, and I’m not just talking about third down. I’m not talking about red zone. Yes, I’m including those two-minute backed up, four-minute. I’m including those, but there’s so many situations within situations. There’s so many things that pop up. And he always, always, always, spent a ton of time talking about those things and practicing those things and walking through those things. And it made us a smarter football team. I do believe that we definitely became a smarter football team because of it, and so I always took that from him.
“And, you know, I always thought he was great with our families, too. My wife can’t wait to see him today to say hi to him, because he was just always great with the families and he made it a family atmosphere there at the Chargers.”
Stupid Observation of the Day: I respect Derek Barnett’s commitment to wearing pants at practice every day. Today was a steamy one but Barnett didn’t waver.

NBA Brooklyn Nets have suspended point guard Kyrie Irving from training until he can play at home in the NBA, according to the team’s website. To do this, the basketball player must be vaccinated against the coronavirus, since New York has tough measures against COVID-19.
The Irving ban does not apply to away matches and training with his team. Irving himself refuses to be vaccinated for religious reasons.
“This is Kairi’s personal choice and we respect his freedom. This choice currently limits his ability to be a permanent member of the team. We will not allow any of our team members to be part-time, ”said Nets General Manager Sean Marks.
Irving is among the top 10 highest paid players of the current NBA season, according to Forbes. Under the terms of the contract, the 29-year-old basketball player can earn more than $ 35 million for the season. Earlier it was reported that Irving would lose about $ 380 thousand for each missed home game.

90,000 Jos Verstappen: Canceling training is not a problem
In Imola, Liberty Media changed the weekend format.Friday sessions have been canceled and Saturday workout will be an hour and a half, not the usual hour. This idea is popular with former Formula 1 driver Jos Verstappen.
Jos Verstappen: “It’s great that Liberty Media is trying to change something. I am satisfied. The shortened racing weekend is a plus for the mechanics, and the race can become more entertaining, because the teams have less time to set up the cars. Thus, you can reduce the gap – then the fight will become more interesting. In Germany, Friday training was canceled due to bad weather, and on Sunday we saw a very interesting race.
Skipping two sessions for an hour and a half is not such a big problem for teams. The main work goes on at the bases, and in training they just dot the i’s. By the start of the weekend, the teams find the optimal settings on the simulator. In my time, everything was different. ”
However, not everyone agrees with the cancellation of Friday training. Former Formula 1 drivers Guido van der Garde and Christian Albers believe that the two-day weekend format will not take root.
Guido van der Garde: “Now the experiment can be successful, because all the teams have already held so many races in a row and entered the rhythm. If the riders complete fewer laps in Imola, this is not a problem for them – after a while, each of them will learn the track. But I doubt that after the cancellation of two workouts, the weekend will become much more interesting, because the main work is going on at the base.
I’m sure this is a one-time experiment. It is no coincidence that the weekend format has not changed for many years, and Friday has always been a busy day. In the first training session, the teams study the track, in the second, they immerse themselves in work: they improve the settings, imitate the qualifications and the race, work with the tires.Without Friday training, all this will have to be done on Saturday. The teams will have to start the Saturday session blind, which is difficult. Better to keep the old format. It’s more interesting for the audience too. Tickets for Friday’s training sessions are much cheaper and the fans see a lot of cars on the track. ”
Christian Albers: “This experiment doesn’t make sense. Formula 1 is about perfecting the car and finding the limit of its capabilities. A car in a wind tunnel is different from what we see on the track. The whole point of Formula 1 is in the fine tuning of the car.
Liberty Media doesn’t know what they are doing. They are shortening the race weekend to keep costs down. But if the car breaks down on Saturday, the whole weekend is ruined. In addition, by canceling Friday’s training sessions, Liberty Media is depriving fans of the opportunity to pay less money to get a closer look at Formula 1. ”
